Placing an abdominal drainage catheter after abdominal surgery is an important means to prevent postoperative complications and treat some surgical diseases, but it is also a controversial technique. On the one hand, the placement of the abdominal drainage catheter is conducive to divert or drain the fluid accumulated in the abdominal cavity (including blood, pus, inflammatory exudate, bile, secretions, etc.); but on the other hand, the abdominal drainage catheter also aggravates digestion Opportunities such as tract fistula, intestinal adhesions, and abdominal infection.

Placement of the drainage catheter

The most important principle of drainage catheter is to place it at the lowest point of the abdominal cavity so that it can fully drain all the fluid in the abdominal cavity.

When lying down, the lowest point of the abdominal cavity is the "Morrison space", "left subdiaphragmatic space" and "Douglas fossa". If there is no barrier, theoretically all of them will flow to these three areas, so the drainage catheter is placed in these three areas, the easiest to let the fluid in the abdomen flow clean.

Color change of drainage catheter

After laparoscopic surgery, a drainage catheter can be placed in the body to observe whether the surgical wound has bleeding, bleeding, and complications. Routine care of the drainage catheter after the operation, first need to observe the color of the drainage catheter.

If the color of the drainage catheter is different from the color at the beginning, there may be lesions in the abdominal cavity. For example, after laparoscopic cholecystectomy, if the color of the drainage fluid changes from pale blood to bile-like, yellow, it means that bile has appeared. Fistula. If after laparoscopic colorectal cancer surgery, the color of the drainage catheter changes from pale blood, pale yellow to fecal watery and smelly, it may be an anastomotic leak.

Summary of nursing care of drainage catheter

1. Secure it properly and mark with a marker on the proximal end.

2. The drainage catheter is clearly marked. Indicate the name of the drainage catheter, indwelling date, time, and length of exposure, and fix the logo on the end of the drainage catheter 2-3 cm (about two fingers wide).

3. Keep the drainage catheter unobstructed, prevent twisting, compression, folding, and avoid the catheter from falling out.

4. Squeeze the drainage catheter every 2 hours. When squeezing, one hand should bend the drainage catheter 10-15cm out of the abdominal skin, and the other hand should hold the proximal drainage catheter in a half-fist shape, that is, the index finger, middle finger, ring finger, little finger and the big thenar are vigorously applied. Squeeze the drainage catheter several times, then release both hands at the same time, and repeat the operation.

5. Observe the color, nature, and quantity of the drainage fluid, and focus on observation within 6 hours after the operation. If the drainage of bloody fluid is more than 200ml per hour, it indicates active bleeding and should be treated immediately.

6. Prevent infection

① Strict aseptic operation when replacing the drainage bag.

②Keep the skin around the drainage catheter clean and dry, and notify the physician in time if there is blood oozing.

③The drainage bag is lower than the puncture hole in the abdominal wall to prevent the reflux of drainage fluid from causing retrograde infection.

④ Replace the drainage bag at least twice a week, and replace it in time when the drainage fluid exceeds 3/4. It is forbidden to open the lower switch of the drainage bag at will to damage the drainage closed environment.

7. Do a good job in education, inform patients and family members of the purpose, importance and precautions of placing drainage catheters, and obtain cooperation to avoid unplanned extubation.
